General Description of Post
The post holder will work as part of a team to provide physical, emotional and recreational care for children with life limiting conditions and their families.
The post holder will work flexibly alongside other members of the care team to respond to the individual needs of the child and family.
Care will be provided either in the child’s home, within “Treetops” or at any other appropriate location identified by the family.
The hours of work will be coordinated by the child’s named nurse and will be flexible to meet the needs of the families.
Key Responsibilities
- Following appropriate training, provide physical, emotional and recreational care for children and their families under the direction of the child’s named nurse and in accordance with an agreed plan of care.
- To work flexibly alongside families, acknowledging their expertise and allowing families to choose the level of involvement they require from the care team, working at all times in partnership with the child and family.
- To have the ability to work without direct supervision whilst recognising that the overall responsibility for the child’s care remains with the named nurse.
- To report regularly to the child’s named nurse and to report immediately any untoward incidents occurring during an episode of care.
- To record in the child’s care documentation an accurate account of care given.
- To maintain confidentiality in all matters pertaining to the child and family.
- To adhere to the Trust’s policies, procedures and guidelines at all times.
- To Promote the Philosophy of The Donna Louise Trust beyond the establishment.
Administrative / Management
- To identify any practice limitations and attend training programmes as directed by your manager.
- To liase closely along agreed lines of communication with the co-ordinator regarding availability for work.
- To keep an accurate record of all hours worked and submit claim forms at agreed times as required by the line manager.
- To participate in group and individual clinical supervision to ensure that personal and team development needs are identified and met.
