Our Community Nursing team provides a bridge from the home to the hospice and is often the family's first point of contact with the hospice. We hope to help the families to cope within their own home environment, and the support is variable, depending on the circumstances within each family, their existing network of support, and the nature and rate of the child's illness.
The Community team provides the following:
- Practical and emotional support for children and their families at home.
- Providing respite and end of life care at home.
- Acting as a link for families and help to access othe services and support.
